If something happens outside of the course of work, the Refrigeration Workers Health & Welfare Plan includes Short-Term and Long-Term Disability Insurance for Class 1, 2, and 12 members.  Short-Term Disability Insurance terminates at age 70.  Long-Term Disability Insurance terminates at age 65.

The first step is to go see a doctor/physician.  If your injury or illness will prevent you from working, contact the UA 516 Health & Welfare Office as soon as possible so that we can get the process underway.  The Health & Welfare Office must prepare a final report to accompany the short-term disability application when it is sent to Canada Life, otherwise the application will be put on hold.  To save time, you can download the file you need: Short Term Disability Application.  You complete your section, the physician completes theirs.

Disability benefit payments are taxable, and the Canada Revenue Agency requires Canada Life to withhold tax from each payment.


Currently, the Short-Term Disability benefit amount is $750 weekly.

Currently, the Long-Term Disability benefit amount is $3,000 monthly.
